Primary Care

The quality measures group has focused on relational continuity as an important principle in their selection of quality measures. They use an iterative process in measure selection to ensure coverage of all dimensions of care.

The integrated activity agreement provides the legal framework for public providers such as health authorities, and private physician practices to exchange identifiable information and enable data-driven improvements to patient outcomes.
